Sample New York quarterly report · high school (9–12) (2026–2027)

New York sets four quarterly report deadlines a year for every home-instruction family, on the dates the IHIP itself set, and the annual assessment is filed with the fourth (8 NYCRR §100.10). Below is a completed example, not a blank form: for grades 9–12 the report tracks the units of study the IHIP planned, at the 990-hour annual equivalent, the same record a transcript is later built from. The student is fictional; the wording is real, and you are welcome to borrow it.

The same engine that rendered this example can fill in your report: families who keep their student's record on Eformogi have each quarterly report draft itself, hours aggregated by subject, and the filled-in preview is free. Capturing the record is free for the learner, forever; generating the clean, filing-ready document is part of Family Pro, $119 a year or $14.99 a month, and each student's first New York IHIP of the season renders free.

Questions families ask

When are New York quarterly reports due?

Four quarterly reports are due on the dates set in the IHIP you filed. The regulation does not set the dates; your own plan does, and the fourth report travels with the annual assessment (8 NYCRR §100.10).

What goes in each quarterly report?

For each subject: the hours of instruction, a description of the material covered, and a grade or a short written narrative of progress, plus a statement about hours if less than 80 percent of the planned hours were completed. The sample above shows each part filled in.

Do I have to use this exact wording?

No. The wording is intentionally general ("regular composition assignments" rather than "one polished essay per month"), which is what district reviewers prefer. Borrow any line and adjust the details to your family.

How many hours should a grades 9–12 report show?

The annual equivalent for grades 9–12 is roughly 990 hours, so a typical quarter shows about a quarter of that, spread across the required subjects. Districts review for the required parts, not for a specific format.
