How to fill it in, step by step
A quarterly report has five parts. The regulation text (8 NYCRR §100.10) is always the source of truth; this is the plain-language version of what to write on the printed form.
1. Student name, grade, and reporting period
The template pre-prints the common window for the quarter you picked. If your IHIP set different dates, write yours in: the dates in your IHIP are the ones that govern, and the report should cover that span.
2. Hours, subject by subject
Enter the hours of instruction for each subject on the ruled line in the hours column, then the total for the quarter at the bottom. You are reporting hours actually taught, not a number promised in advance; across the year the four totals add up to the annual equivalent (900 hours for grades 1 to 6, 990 for grades 7 to 12).
3. Material covered in each subject
A short description of what was actually covered this quarter: the topics, chapters, or units. A couple of plain sentences per subject is normal, and the wording from your IHIP's plan of instruction carries naturally into the past tense here.
4. A grade or a written narrative per subject
Either one satisfies the regulation. A narrative, a few honest sentences about what the student did and how it went, is a full substitute for a letter grade.
5. The 80 percent note, then file it
If less than 80 percent of the material planned for a subject was covered, add a brief written explanation beside that subject; it is a routine note, not a penalty.
